Form with 3 types of questions: Multiple choice, yes no and fill result

This form has 3 parts corresponding to 3 types of questions:
1. Multiple choice question, choose 1 of 4 options A, B, C, D
2. True or false questions: Each sentence will have 4 ideas a, b, c, d with each idea the student chooses true or false. The score of the sentence will correspond to the number of correct ideas in the student's sentence.
3. Fill-in question: Students calculate the result into a number and then fill in the result

The answer sheet sample combines multiple choice and essay. This form has 2 parts:
+ Part 1 can have up to 40 multiple choice questions
+Part 2 There is an essay space for students to write, the teacher grades and adds points
The software automatically scores multiple choice tests and synthesizes them with the teacher's essay scores to produce results
